Model Releases

Model Releases must be completed and signed by every person in each clip. We need a separate Model Release completed for each person and each shoot. All signatures must be handwritten and dated with a pen – digital signatures or copy/paste will not be accepted. All releases must be on our unmodified single-page Model Release form and must have ALL of the following information completed and be totally legible:

In the “Photographer/Filmmaker/Shoot Information” section, upper right corner:
*REQUIRED INFO

  • *filmmaker full legal name (print)
  • *filmmaker signature – must be handwritten with a pen – digital signatures or copy/paste will not be accepted
  • *date signed – handwritten with a pen
  • *shoot date – must include a specific, complete and clearly noted shoot date. Words such as “various” or “several” or whole months are not acceptable. If there was more than one shoot, we need a separate release for each shoot.
  • *shoot country
  • *shoot state/province
  • shoot description/ref. – PLEASE LEAVE THIS BLANK (unless shoot contains any nudity or is suggestive)

In the “Model Information” / “Model Contact Info” sections, middle right side:
*REQUIRED INFO

  • *model full legal name (print)
  • *model date of birth
  • *model residence address, city, state, country, zip
  • *model phone and email
  • *model signature – must be handwritten with a pen – digital signatures or copy/paste will not be accepted
  • *date signed – handwritten with a pen
  • *model gender and race/ethnicity
We also need a clear *photograph of the model (or screen shot from video) showing their face and put in upper right corner (aligned to top right-hand corner if larger than box). For example, ID, snapshot, screen shot from video, etc. 

If the model is a minor (under 18 years old in the US), the parent name and information must also be completed and the parent must sign and date the Model Release. If the same model is used in more than one shoot, we need a separate release for each shoot.

After the Model Releases are completed and signed, they need to be scanned (at least 200 ppi, clear, clean and straight) and saved as “.jpg” files (with this exact suffix) so they can be sent to us along with signed copies of government photo identifications for each model.  We have model release forms available in many languages. If you would like one in a language other than English, just let us know!
